Helps the patient and their family by linking them with resources in the hospital and in their own community. Shall also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

DUTIES AND RESPONSIBILITIES:

• Evaluates patient's economic condition as to their capacity to pay the hospital bills.
• Identifies people who need help.
• Interviews patient and their family members for their discharge planning.
• Interviews and prepares social case study report for possible referral of patient and family to other hospitals and agencies for availment of services not catered by the hospital.
• Facilitates networking with other hospitals, GO's and NGO's for possible assistance.
• Researches and refers the patient to community resources.
• Helps the patient work with government agencies to apply for and receive benefits such as Medicare.
• Follow up with the patient to ensure that their situations have improved.
• Evaluates services provided to ensure that they are effective.
• Provides guidance and counseling to patient and/or relatives.
• Conducts home visits and collateral units to patients and his family whenever necessary.

Candidates must be:

• A graduate of BS Social Worker and/or related field
• Must be licensed/registered Social Worker in the Philippines
• With good organizational skills and critical thinking
• With good communication and writing skills
