Position Summary

We are seeking a highly skilled and compassionate Remote Registered Nurse (RN) to join our client's clinical workforce team. In this fully remote role, you will work under the direction of our physician partners to provide high-quality patient outreach, care coordination, and clinical support.

The ideal candidate possesses excellent communication skills, strict attention to detail, and a deep understanding of clinical workflows. You will serve as a vital link between patients and their primary care or specialist teams, ensuring seamless transitions of care, adherence to treatment plans, and timely escalation of medical concerns.

Key Responsibilities

  • Physician-Directed Patient Outreach: Initiate proactive phone calls and secure messages to patients based on physician orders, care plans, or population health initiatives.
  • Clinical Follow-Up: Conduct post-visit, post-discharge, and routine follow-up assessments to monitor patient recovery, symptom management, and treatment adherence. The RN should have ability to prioritize based on practice protocols: RN Critical Telephone Encounter (TE) Alerts, RN TE based on date received, TCM, RPM and other work as assigned in ECW based on practice needs
  • Patient Education: Translate complex medical information into easily understandable terms, educating patients and their caregivers on disease management, medication regimens, lifestyle modifications, and preventative care.
  • Comprehensive Documentation: Accurately and concurrently document all patient interactions, clinical assessments, and care plan updates within the Electronic Health Record (EHR) system in strict compliance with clinical guidelines.
  • Escalation Support: Perform clinical triage during outreach, rapidly identifying changes in patient status, red-flag symptoms, or medication adverse effects, and properly escalating issues to the directing physician or emergency services as required.
  • Care Coordination: Collaborate with the broader interdisciplinary team, including physicians, pharmacists, and medical assistants, to ensure all patient care needs and inquiries are fully addressed.

Qualifications & Requirements

  • Education: Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) strongly preferred.
  • Licensure: Active, unencumbered NY-RN license. Note: Multi-state Compact (eNLC) license or willingness to obtain additional state licenses is highly desirable.
  • Experience: Minimum of 2-3 years of recent clinical nursing experience (e.g., med-surg, primary care, emergency, or case management). Prior experience in specialty of nephrology (kidney care, dialysis, medical office care, and lab results,)
  • Technical Proficiency: Strong computer skills, including experience navigating various Electronic Health Record (EHR/EMR) systems such as eClinical Works (eCW) EMR, secure messaging platforms, and telehealth software.
  • Communication: Exceptional verbal and written communication skills with a professional, empathetic, and patient-centric phone demeanor.
  • Critical Thinking: Ability to work independently, think critically, and make sound clinical judgments based on physician protocols and standing orders.
