Job Summary

The Medical Records Officer is responsible for maintaining, organizing, securing, and managing patient medical records within the oncology or chemotherapy center. The role ensures accurate documentation, confidentiality of patient information, timely retrieval of records, and compliance with healthcare regulations and documentation standards.

Key Responsibilities

Medical Records Management

  • Maintain complete, accurate, and updated patient medical records.
  • Organize, file, scan, and archive medical documents systematically.
  • Ensure proper encoding and updating of patient information.
  • Retrieve patient records promptly upon authorized request.

Documentation & Data Accuracy

  • Review medical records for completeness, accuracy, and proper documentation.
  • Monitor submission of treatment records, laboratory results, physician notes, and consent forms.
  • Coordinate with healthcare providers regarding incomplete or missing documentation.

Confidentiality & Compliance

  • Ensure confidentiality and security of patient records in compliance with data privacy laws and healthcare regulations.
  • Control access to medical records and release information only to authorized personnel.
  • Participate in audits and compliance checks related to medical documentation.

Coordination & Administrative Support

  • Coordinate with physicians, nurses, billing staff, and other departments regarding patient documentation needs.
  • Assist in preparing records for insurance, HMO, PhilHealth, and legal purposes when authorized.
  • Support report generation and statistical data collection for management and regulatory requirements.

Records Retention & Filing

  • Maintain proper storage, retention, and disposal procedures for medical records.
  • Ensure backup and protection of electronic and physical records.
  • Monitor filing systems to improve efficiency and accessibility.

Qualifications

  • Bachelor’s degree (IT, Health Information, or related)
  • 1–2 years of experience in records/IT
  • Knowledge of data privacy and records management
  • Computer literate / system maintenance skills
  • Organized and detail-oriented
  • Preferably residing or near San Mateo
