About the Role:
We are seeking an experienced RPM-CCM Team Lead to oversee Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. This role involves monitoring patient health data, conducting monthly outreach and follow-up calls, completing patient assessments, coordinating care and referrals, educating patients, and ensuring accurate EHR documentation and billing compliance.

The Team Lead will work closely with healthcare providers to address patient needs, escalate concerns, and support quality patient care while ensuring compliance with Medicare, payer guidelines, and HIPAA standards.

Qualifications:
- Experience in RPM and/or CCM programs, preferably in a leadership role.
- Hands-on experience with eClinicalWorks (eCW) is required.
- Strong patient engagement, communication, and organizational skills.
- Knowledge of EHR documentation, billing compliance, and HIPAA.
- Ability to monitor patient data, coordinate care, and manage a remote team.
