Full job description

Portiva started in 2009 with our mission to help Doctors and Medical Practitioners manage their practices better. Since then, Portiva has grown from a 3-person operation to one of the largest and most successful Medical and Dental Staff Providing Companies in the United States.

We are looking for an experienced Medical Coder with strong knowledge of Medicare, ICD-10-CM, PDGM, and medical documentation requirements.

QUALIFICATIONS:

  • Minimum 2 years U.S. medical coding experience
  • Direct Home Health coding experience
  • Experience with Medicare-certified Home Health agencies preferred
  • Strong knowledge of ICD-10-CM, diagnosis sequencing, and PDGM
  • Experience reviewing OASIS and Home Health clinical documentation
  • Familiarity with Home Health EMRs such as HCHB, WellSky, Axxess, or similar
  • Coding certification such as CPC, CCS, CCS-P, or CCA is a plus

KEY RESPONSIBILITIES:

  • Review Home Health records and clinical documentation
  • Assign accurate ICD-10-CM diagnosis codes
  • Handle primary and secondary diagnosis sequencing
  • Review coding for SOC, ROC, Recertification, and other Home Health episodes
  • Review OASIS and supporting documentation
  • Identify documentation gaps and coding discrepancies
  • Ensure compliance with Medicare, CMS, and PDGM guidelines
