About the Role

We are seeking an experienced and passionate Care Partner to join our team supporting a growing Australian based national aged care provider. The Care Partner role is responsible for coordinating, leading the delivery of high quality, care management services through the Support at Home Program for Australian based senior care participants.

Working closely with participants, families and various health professionals you ensure services reflect each participant's assessed needs, preferences and available funding. The Care Partner is the participant's primary contact for care management and service coordination which involves monitoring care plans, budgets, service quality and outcomes.

The purpose of the Care Partner position is to provide quality, day-to-day support to management and staff. This is a key role for someone who thrives in supporting both our participants and our organisation through the changing aged care landscape.

Key Responsibilities

• Coordinate agreed care and services for participants receiving support in their homes.
• Develop and regularly review care plans with participants, families and health professionals.
• Act as the main contact for participants, responding promptly to questions, concerns and changing needs.
• Provide the roster team with clear, current information about participant needs, preferences and service changes.
• Work with support workers, care management team and external health professionals to ensure consistent service delivery.
• Monitor participant budgets, expenditure and service utilisation to address potential surplus or deficits.
• Maintain accurate and current participant records, care plans and service information.
• Identify and respond to risks, incidents and safety concerns, escalating matters in accordance with company procedures.
• Seek participant feedback, respond to service concerns and contribute to continuous improvement.
• Comply with relevant aged care quality standards and organisational policies.

Qualifications

• Minimum 2+ years experience with Home Care Packages / Support at Home and compliance requirements
• Diploma or tertiary level qualification in Community Services, Aged Care, Case Management
• Background in allied health (Registered Nurse (AHPRA) / Physiotherapist / Allied Health Professional) desired but not necessary 
• Previous experience in case management, clinical coordinator, or experience in home care management highly regarded
• Knowledge of home care regulations, industry standards, and best practices.
• Strong communication skills with the ability to interact effectively with people at all levels.
• Sound understanding of compliance in Aged Care and NDIS (including the quality standards, WHS, infection prevention and control, and SIRS) setting
• Understanding of the care needs of the elderly or individuals with disabilities.
• Experience working collaboratively with a dedicated rostering or scheduling team
• Excellent interpersonal leadership and time management skills
• Compassionate, adaptable, and confident, working autonomously

Salary Package

• From PHP70,000 per Month based on candidate skills & experience

Pay: From Php70,000.00 per month
