About the Role

We are seeking a detail-oriented and highly organized Clinical Documentation Specialist to join our growing team. In this remote role, you will support healthcare providers by ensuring patient medical records are accurate, complete, and compliant with healthcare documentation standards.

The ideal candidate has experience in medical documentation or medical scribing, a strong understanding of medical terminology, and proficiency with Electronic Health Record (EHR) systems. Key Responsibilities

  • Prepare and maintain accurate clinical documentation, including treatment plans, ASAM assessments, intake assessments, group notes, individual progress notes, and discharge summaries.
  • Document patient encounters, medical histories, clinical findings, treatment plans, and follow-up care during or after consultations.
  • Enter laboratory results, diagnostic reports, and other relevant clinical information into the Electronic Health Record (EHR) system.
  • Update patient records promptly with new diagnoses, medications, treatments, and follow-up plans.
  • Assist with assigning appropriate medical codes based on documented clinical information.
  • Review documentation for accuracy, completeness, consistency, and compliance with healthcare documentation standards.
  • Ensure all patient records comply with applicable healthcare regulations and privacy requirements, including HIPAA.
  • Support internal and external audits by organizing and providing accurate clinical documentation as needed.
  • Coordinate and document patient follow-up activities, including appointment reminders, medication renewals, and other clinical support tasks.
  • Communicate and accurately document patient care instructions to ensure continuity of care.

Qualifications

  • Previous experience in medical documentation, medical scribing, or a similar healthcare support role.
  • Strong knowledge of medical terminology and clinical documentation practices.
  • Hands-on experience using Electronic Health Record (EHR) systems.
  • Excellent written communication skills with exceptional attention to detail.
  • Ability to handle confidential patient information with professionalism and discretion.
  • Strong organizational, time management, and multitasking abilities.
  • Ability to work independently in a remote environment while consistently meeting deadlines.
