DUTIES AND RESPONSIBILITIES:

· Routinely checks for the existence of any prior medical records of every patient admitted as an inpatient or as an outpatient.

· Retrieves any prior record for inclusion with current record.

· Regularly retrieves records of patients scheduled for admission to clinic or hospital.

· Follows established procedures for dispatching medical records to other areas of the hospital.

· Updates patient’s index file by filing new cards or recording additional data on existing cards and audits patient’s index for misfiles.

· Files cards according to established filing system.

· Files medical records by established numbering system and rechecks filing for proper location on filing shelves.

· Replaces worn or torn folders and may issue file folder on new admissions and dispatches them to patient care areas.

· Performs other tasks that may be assigned by the superior from time to time.
