Education : Graduate of Bachelor of Science in Nursing

Experience : At least one (1) year of related work experience but not required, fresh graduates may be considered. Must be a Registered Nurse (RN)

Training : Basic Life Support, Advanced Cardiac Life Support, Customer Service Training

DUTIES AND RESPONSIBILITIES

Safe and Quality Nursing Care

  • Identifies patients correctly using two (2) Patient Identifiers.
  • Effectively observes policies on safe handling and administration of High-Alert Medications (HAM), sound-alike and look-alike medications.
  • Secures specimens safely and labels accurately and correctly.
  • Performs 5 moments of hand hygiene as may be indicated.
  • Assesses patients for risk of fall and implements fall protocols accordingly.
  • Admits patients promptly based on the admission process.
  • Transfers/transports patients safely to designated areas and hands him to the receiving unit.
  • Carries discharge orders promptly and effectively communicates health education and discharge instructions to patients and relatives.
  • Performs initial patient assessment and reassessment using the prescribed form.
  • Provides uniform care to all patients that meet their safety, comfort and hygiene needs.
  • Provides specialized care for specific high risk populations.
  • Performs the procedures on transcription process and safe drug administration.
  • Performs basic infection control measures thru hand hygiene, use of PPE’s, proper waste disposal, observance of standard/transmission-based precautions and observance of bundles of care.
  • Responds to disaster calls appropriately.
  • Safely operates biomedical equipment necessary for patient care.
  • Performs end-of-life care according to hospital protocol. End-of-life care may include palliative care, supportive care and hospice care.
  • Performs post-mortem care to expired patients according to hospital protocol.

Communication

  • Reads back and verifies received orders from authorized physicians according to the procedure on written, text and phone orders.
  • Effectively endorses/hands over pertinent patient care data for continuity of care in every shift to shift endorsement from General Nursing Section to Specialized Area and vice versa; and General Nursing Section/Specialized Area to ancillary and vice versa.
  • Effectively utilizes ISBAR tool when giving situational briefing to doctors.
  • Recognizes and reports notifiable diseases under Republic Act No. 11332.
  • Recognizes and reports communicable diseases under Republic Act No. 3573.

Collaboration and Teamwork

  • Recognizes the members of the healthcare team and understands their expectations from the nurses.
  • Effectively coordinates and collaborates with physicians through enhanced professional interaction.

Management of Resources and Environment

  • Charges and replaces all the supplies used.
  • Observes the borrowing system.
  • Keeps his workplace clean and organized.
  • Economizes office supplies.

Health Education

  • Gives health education according to the patient/relatives learning needs.
  • Ensures that confidentiality and privacy is provided in a respectful manner at all times.

Personal and Professional Growth

  • Attends activities pertaining to his professional development.

Records Management

  • Data integrity is maintained and all information is kept with utmost confidentiality.
  • Nursing documentation in the patient’s medical records are completed accordingly.

Quality Improvement

  • Understands and delivers the key performance indicators expected from him.
  • Contributes for quality improvement projects.

Customer Care

  • Demonstrates attentiveness to customers and sensitivity to their healthcare needs.

Leadership at the Point of Care

  • Demonstrate skills at the first level of clinical decision making.
  • Effectively manages care of patients at the bedside.

Other Functions

  • Performs job functions consistent with the vision and mission statements and the core values of the hospital.
  • Attends training programs and learning sessions for the enhancement of professional, technical, and personal proficiencies.
  • Maintains comprehensive knowledge of quality management systems (QMS) principles and practices while actively participating in all QMS-related activities, including audits, training sessions, and seminars, to contribute to continuous improvement initiatives.
  • Performs such other duties and responsibilities analogous, germane or related to and/or implied from the above enumeration of duties and responsibilities.
