Job Overview

Hygieia Staffing and Placement Services is seeking an experienced Home Health ICD-10 Coder to join our growing clinical documentation and quality assurance team.

The Home Health ICD-10 Coder will be responsible for reviewing clinical documentation, assigning accurate ICD-10-CM diagnosis codes, validating coding compliance, and supporting OASIS documentation to ensure accurate reimbursement and compliance with CMS guidelines.

The ideal candidate has strong knowledge of home health regulations, OASIS documentation, PDGM requirements, and ICD-10-CM coding guidelines.

ResponsibilitiesCoding & Documentation Review

• Assign accurate ICD-10-CM diagnosis codes for Home Health patients.
• Review Start of Care (SOC), Recertification (RECERT), Resumption of Care (ROC), Transfer, and Discharge documentation.
• Validate primary and secondary diagnoses based on physician documentation and CMS guidelines.
• Review OASIS assessments for coding consistency and accuracy.
• Ensure coding supports PDGM reimbursement requirements.
• Review physician orders, medication profiles, and supporting clinical documentation.
• Identify documentation deficiencies and communicate coding recommendations to the QA or clinical team.
• Maintain coding accuracy in accordance with current ICD-10-CM Official Guidelines for Coding and Reporting.
• Assist with coding corrections and documentation clarification when necessary.
• Support quality assurance initiatives and coding audits.

Compliance

• Ensure compliance with CMS, Medicare, PDGM, and payer-specific coding requirements.
• Maintain HIPAA compliance and confidentiality of all protected health information.
• Stay current with annual ICD-10 coding updates and CMS regulatory changes.

QualificationsRequired

• Certified ICD-10 Coder (CPC, CCS, COS-C, HCS-D, BCHH-C, or equivalent certification preferred).
• Minimum of 2 years of Home Health ICD-10 coding experience.
• Strong understanding of:
• Home Health regulations
• OASIS documentation
• PDGM reimbursement methodology
• ICD-10-CM Official Coding Guidelines
• Medicare Home Health Conditions of Participation
• Experience reviewing physician documentation and Plan of Care (485/POC).
• Excellent analytical and problem-solving skills.
• Strong attention to detail and documentation accuracy.
• Excellent written and verbal communication skills.
• Reliable internet connection and dedicated remote workspace.

Preferred

• Experience with HomeHealthMD.
• Experience with WellSky/Kinnser, MatrixCare, Axxess, or other Home Health EMR systems.
• QA or OASIS review experience.
• Familiarity with Medicare ADR documentation requirements.
• Experience working with multi-state Home Health agencies.
