Position: Medical Records Assistant
Location: Blk 1 Lot 33 Springfields Residences, Brgy. Longos, City of Malolos, Bulacan & Assigned Onsite Locations
Type: Full-Time, Probationary
Reports To: Chief Operating Officer

Role Overview
The Medical Records Assistant at TeleCure plays a key role in maintaining accurate, organized, and confidential patient medical records in support of clinical operations, reporting, and compliance. This position ensures that all patient information—demographic, clinical, and administrative—is properly encoded, updated, and validated within the clinic’s information systems and applicable government health platforms, including PhilHealth. The role contributes directly to the efficiency of service delivery, supports onsite activities such as health caravans, and upholds the integrity of data used for institutional monitoring and participation in government-supported health programs.

Duties and Responsibilities

Patient Records and Data Management

  • Encode and update patient demographic details, medical histories, and service records in the clinic’s database, electronic medical record (EMR) system, and PhilHealth or other government health information systems.
  • Ensure accuracy, completeness, and proper categorization of both active and archived patient records.
  • Support record population and maintenance activities required for compliance, monitoring, and reporting.
  • Assist in the registration, documentation, and data encoding of patients during onsite activities such as health caravans.

Medical Services Documentation

  • Encode details of consultations, diagnostics, medications, and treatments rendered.
  • Record all data accurately based on verified documentation from attending physicians and healthcare staff.
  • Assist in the consolidation of historical or backlog data to ensure complete patient information.
  • Facilitate the secure and documented release of laboratory, diagnostic, and medical results to patients or authorized representatives in accordance with clinic protocols and the Data Privacy Act of 2012.
  • Ensure that all released results are properly logged, verified, and filed for audit and compliance purposes.

Data Validation and Quality Assurance

  • Cross-check encoded data with original documents to ensure accuracy and consistency.
  • Participate in data validation, correction, and reconciliation activities for internal audits and external reviews.
  • Identify and report discrepancies for timely resolution.

Reporting and Coordination

  • Generate regular and ad hoc reports for management review and submission to relevant agencies or partner institutions, including PhilHealth.
  • Coordinate with healthcare providers and administrative staff to clarify incomplete or inconsistent records.
  • Support claims documentation and respond to record-related queries from internal and external stakeholders.
  • Facilitate timely and accurate submission of reports and data to appropriate government and partner entities.

Compliance and Confidentiality

  • Maintain patient confidentiality in accordance with the Data Privacy Act of 2012 and organizational policies.
  • Ensure that all recordkeeping procedures comply with applicable health regulations and institutional standards.
  • Maintain orderly and secure storage of both digital and physical medical records.

General and Administrative Support

  • Maintain the organization, cleanliness, and security of the medical records section.
  • Assist in improving systems and workflows related to medical documentation and records management.
  • Perform other duties as may be assigned by Management to support operational efficiency and compliance requirements.

Qualifications

  • Bachelor’s degree in Health Information Management, Nursing, Medical Technology, or any related field.
  • Fresh graduates are welcome to apply. Prior experience in medical records management or healthcare data entry is an advantage.
  • Basic knowledge of medical terminology and healthcare documentation preferred.
  • Proficient in Microsoft Office and data entry systems.
  • Strong attention to detail, accuracy, and organization.
  • Excellent communication and coordination skills.
  • Trustworthy, dependable, and capable of handling confidential information.
  • Willing to learn, adapt, and contribute to continuous improvement in operations.

Work Schedule

Days: 6 days a week

Hours: 8 hours per day
